  • Coordinate, implement and evaluate person-centered community leisure and volunteer activities on a daily basis.

• • Manage daily operation of Snack Shack, including but not limited to tracking inventory, sales, profit/loss, etc.
  • Assist participants in planning appropriate community integration activities consistent with accessibility of community site and classes.
  • Plan and implement written lesson plans and activities for the individuals enrolled in the course consistent with participant needs and valued outcomes as defined in ISP.
  • Integrate activities that can be accomplished during Community Inclusion that will augment the lessons in the classroom.
  • Facilitate integration to each site, encourage participant participation in dialogue, and solicit feedback consistent with learning module to enhance learning and awareness.
  • Maintain daily attendance record and time sheet.
  • Order necessary supplies and learning aids to augment curriculum.
  • Assist whenever needed with the management needs of the participant including, but not limited to, ADL as well as behavioral interventions.
  • Monitor and document response to valued outcomes and informal outcomes as required.
  • Maintain bi-weekly communication with the Life Advisor regarding participant needs and progress.

• Develop and maintain consumer schedule based upon consumer choice as identified in ISP, through valued outcomes, team, and family/guardian input.

• Develop IPP and valued outcomes/informal outcomes for all consumers.
• Conduct meetings with groups of consumers to develop ideas for future learning modules.
• Provide guidance and counseling to consumers in relation to learning modules/and the consumer’s goals and administer pre-tests for selected learning modules.
• Refer consumers for clinical evaluations as needed.
• Conduct Annual and Semi-Annual Case Conference meetings.

• Presents Article 16 clinic services to potential people through the designated intake procedure.
• Coordinates the provision of all treatments, activities or therapies as prescribed by the Article 16 clinic admission procedure and by the treating professionals in the patients’ treatment plan.
• Coordinate the inclusion of ISP’s, IPP’s, IEP’s and clinic treatment plans for services delivered by other clinics into the patients’ clinical record.

• Maintain open communication with supervising physical therapist regarding residents’ response to treatment and changes in status.

• Work with other departments to ensure seamless coordination of service delivery.
• Act as a resource for consumers, families and staff in regard to clinical programs by encouraging their participation in the therapeutic process through shared decision making and training.
• Train staff in carry-over activities and safety procedures related to the therapeutic process.

• Provide nursing care and treatment for emergency and minor malady relief.  This includes, but is not limited to, first aid and treatment, changing dressings, and administering medications.
• Act as primary contact with physicians, pharmacies, labs, hospitals (for residents’ treatments, as deemed necessary), day program nurses, etc. and monitor all appointments and follow-ups.
• Implement special health, grooming and personal hygiene programs for the residents.
• Coordinate with physician and pharmacy regarding residents’ monthly medications.

Friday-Sunday and Saturday-Monday (Flexible hours)

Including, but not limited to:

• Work with Recreation Therapist to design and be responsible to implement therapeutic activities.
• Assist in completion of daily activity log and community outing logs.
• Run individual and group therapeutic recreational activities as designed by the recreation therapist.

• Maintain all recreation areas and supplies in clean organized manner with appropriate supplies.

Required: Associate’s Degree in a Human Service field or 3 years experience in working with individuals with developmentaldisabilities. Valid New York State Driver’s License. 1 year of supervisory experience.

• Supervise direct care staff including but not limited to, scheduling (while on duty), providing performance feedback, evaluations and corrective actions.
• Must be willing to travel to multiple sites using own vehicle.
• Monitor the condition of the physical plant, equipment and materials utilized by the program.
• Monitor and ensure compliance with agency policies and all applicable Federal, State, and County regulations.
• Participate in the Interdisciplinary Treatment Team planning process.
• Coordinate and schedule appropriate leisure activities.
• Prepare monthly reports as needed to ensure compliance with all departmental policies.
